Building on our report, A vision for population health: towards a healthier future, this event provides practical guidance on how you can implement the framework in your local area and turn a vision for population health into a reality. In anticipation of the long-term plan placing a renewed focus on prevention and moving beyond integrated care, the need for action is now. The event will provide insight into national action, including the Department of Health and Social Care’s Green Paper on prevention, as well as local examples of how to connect the four pillars of our health: the wider determinants, our behaviours and lifestyles, an integrated health and care system, and the places and communities in which we live.
