The Reducing Risk of Long Term Conditions Level 5 module is aimed at students who have the opportunity to influence the health of individuals, groups and communities. It will enable students to develop the skills, knowledge and confidence to support individuals to make healthy lifestyle choices in relation to long term conditions and to understand the wider implications of the social determinants of health, choice architecture and large scale behaviour change. This module addresses the changing needs of the health and social care workforce and will create greater flexibility and opportunity for future students.Successful students will be able to demonstrate an understanding of the evidence base and policy initiatives informing health and well-being and the prevention of long term conditions. They will be able to identify how health and social care professionals can influence health behaviours at individual, community and national levels.
